STATEMENT OF THE JOB:
Under the direction of the Program Manager, the Case Manager II determines the client needs for assistance, arranges for services, provides information and referral and assists the Program Manager to plan and coordinate education/recreational activities for the Program.

St. Louis Senior Center / 230 Riverdale Ave, Brooklyn, NY 11212
Glenwood Senior Center / 5701 Avenue H, Brooklyn, NY 11234
St. Charles Jubilee Senior Center / 55 Pierrepont St, Brooklyn, NY 11201
QUALIFICATIONS:
High School Diploma/Official Transcript.
Bilingual Spanish/English; Russian/English; Chinese/English; Korean/English speaking preferred.
Able to travel to multiple locations within the five boroughs as needed.
Frequently lifts and/or moves up to 10 pounds.
